Test Entry Form (pdf)

Submission of the test entry form and entry fee is required in order to hold your space. Please do not email the form — print it out and mail it with your check. You will receive an email confirmation after your complete entry is received.

If you are waiting for your dog’s NAVHDA registration number, please send your entry form and fee and you may email the registration number as soon as you have it.

Membership in NAVHDA International is required for you to enter any NAVHDA test; this is in addition to any NAVHDA Chapter membership you may also have. To join or renew your NAVHDA International membership online, click here.

Run day and running order are assigned on a first-come, first-served basis, while also taking into account efficient test planning based on the types of entries received. We will do our best to accommodate your run day preference but cannot guarantee it.

If you have questions about your test entry or the testing process, please email annbagnell@gmail.com.

Waiting List:  We maintain a waiting list when our test is full, but you must send a completed test entry form and entry fee to the Test Secretary (above) to be on the list.

Refund Policy:  Entries withdrawn 45 days before the test date will receive a refund of the test entry minus $25.00. After the 45-day period, no refunds will be made. If your dog is unable to complete the test because of a medical condition or death, a written note from your veterinarian must be provided for a full refund.
